In Home Care Services

We are a community based local charity accredited by the Australian Charities and Not for Profits Commission.– our vision is Connected, Caring and Supportive Communities. We are an Approved Provider of Home Care Packages under the Commonwealth Aged Care Act 1997 and are accredited against the Home Care Standards. We have been coordinating and providing care services to support frail older people to remain living at home independently for over 30 years.

As an aged care provider, we are committed to positive ageing, providing quality support and person centred care that is flexible and responsive to the individual needs of each person.


Our aged care services are provided under three service models: 

Home Care Packages Program

Commonwealth Home Support Program (CHSP)

Private Services

Home Care Packages

A Home Care Package is an allocation of funds (subsidy) by the Australian Government that allows you to tailor a package of care and services to best support your needs and help achieve your goals. Packages are flexible so you can tailor them to meet your needs today and alter them in the future as your needs change.


There are four levels of Home Care Packages, each of which receive a different subsidy.

• Level 1 – Supports people with basic care needs

• Level 2 – Supports people with low level care needs 

• Level 3 – Supports people with intermediate care needs

• Level 4 – Supports people with high level care needs


Home Care Package Fees


All Home Care Packages with CareWays will be delivered on a Consumer Directed Care (CDC) basis. CDC is a partnership approach that offers you choice and flexibility. CareWays will work with you to plan and design a package of care and support that meets your individual needs.


With a CDC Home Care Package, you can:

 

  • Choose the services and products that will best meet your needs
  • Have a greater say in how, when and who delivers your services
  • Work with CareWays to design your support plan
  • Decide how much involvement you have in managing your budget and services

 

Services you can access under a Home Care Package may include, but are not limited to:

 

  • Personal services
  • Cleaning
  • Lawn and garden maintenance
  • Laundry
  • Meal preparation
  • Shopping assistance
  • Social support and outings
  • Respite
  • Medication prompting
  • Transport
  • Personal alarms
  • Mobility aids 
  • Continence aids
  • Nursing, health and therapy services 
  • Home maintenance
  • Window Cleaning

 

The Home Care Packages provided by CareWays are supported by the Australian Government Department of Health. To be eligible for a package, you must first be assessed by the Aged Care Assessment Team (ACAT). To organise an assessment, contact My Aged Care on 1800 200 422.

Commonwealth Home Support Program (CHSP)
CareWays CHSP services provide low to medium level home support for the frail older people and their carers. Services provided by CareWays under the CHSP include:

Food Service - Meals on Wheels

CareWays offers a range of frozen meals delivered to your home -  either through a funded service or for a fee.


Personal Care

Support with daily tasks of self-care to help maintain health and hygiene. Can include assistance with showering, dressing, grooming and the self-administration of medication.

Domestic Assistance

Support with general house cleaning and laundry. Can also include unaccompanied shopping delivered to your home.

Respite Care
In home respite to give carers ‘time out’ to have a break while our staff and/or volunteer provide care for your loved one.

Social Support
In home social visits to offer companionship and support. Can include assistance with letter writing, friendly visiting and social activities.
