An exciting opportunity exists for a CHSP Care Partner to join our Community Care Team, based in Dapto NSW.
About the role
This position provides an important client facing and referral role conducting assessments and completing support plans to register new Support at Home and CHSP clients in a timely manner and in consultation with the Scheduling Officer and Services Team.
This is a permanent position reporting to the Aged Care Senior Manager, working 18-20 hours per week with days and times negotiable with the right candidate.
The successful candidate
To be considered you must have:
- Certificate III or IV In Home and Community Care and/or Disability or equivalent qualification or 5 years relevant experience
- One year experience in a similar position
- Experience in a supervisory or team leader position
- Experience in completing CHSP & HCP assessments
- Accurate and high level of administrative skills with intermediate level of computer proficiency, including use of Microsoft Office Suites and excellent data entry skills
- High level of attention to detail and excellent written and verbal communication skills
- Smart phone and reliable vehicle.

Remuneration
- Position is subject to award conditions under the Social, Community, Home Care and Disability Services Industry Award 2010 as Social & Community Employee Grade 3.
- Uniform and laundering allowance provided as well as mobile phone allowance
